Foreign Resident Registration
New application or Renewal and Notification of Change in Address are accepted at the Citizens Affairs Section.
In place of a Japanese Certificate of Residence，foreigners are issued with   a  Certificate of  Registration Items.

Initial　Registration
(New  application)
(1)Person to apply
Applicant in person, A person(age 16 or above)in the same household  for an applicant under the age 16
(2)Necessary documents
A: Passport
B: Photograph (4.5x3.5cm,taken within the last 6 months)
Not required for children under 16.

Renewal
(1)Application Period
Within 30 days starting from the current registration expires.
Note: The expiration date is shown on your Foreign Resident Registration Card
(2)Person to apply
 Applicant  in person
(3) Necessary documents
A: Passport (if you have one)
B: Photograph (4.5 x 3.5 cm)
C: Foreign Resident Registration Card

Reissues (in case of loss or theft of your Foreign Resident Registration Card )
(1)Application Period
Within 14 days of discovery.
(2)Person to apply
Applicant in person, A person (age 16 or above) in the same household for an applicant under the age 16
(3) Necessary documents
A: Passport. (if you have one)
B: two photographs (4.5 x 3.5 cm) 
    Not required for children under the age 16．
 
Notification of Change in status of residence or period of stay
(1)Notification Period
 Within 14 days of the change.
(2)Person to notify
Person in question, Family member (age 16 or above) in the same household.
(3) Necessary documents
A: Foreign Resident Registration Card
B: Passport
C: Items showing proof of the  change
D: Identification of person making the notification

Notification of Change in address(into/within Chouf City)
(1)Application Period
Within 14days after moving to the new address.
(2) Person to notify
Person in question, Family member (age16 or above) in the same household.
(3) Necessary documents
Foreign Resident Registration Card

Move away from Chouf City
In case of moving away from Choufu City , notification is unnecessary.
But you must notify the government office in your new place of residence.
Note: The insured of the National Health Insurance should come to
the National Health Insurance Pension Section and return the Health Insurance Card.
